The national footprint of DPC practices is growing, with many of these, including our own, already partnering with mostly small, local employers. By creating a unified collective of local, independent DPC practices grounded in a basic and consistent framework of physician-led best practices, we together become a viable option for larger employers, particularly those with multiple locations, higher functioning employer sponsored health plans, or a distributed workforce.

With more than 50% of Americans receiving health care benefits through their employer and employers and benefits advisors increasingly seeking out options to incorporate DPC as part of their benefit plan offering, the opportunity is significant to care for more patients and help create system-level change at the same time.

From early roots in North Carolina and Indiana, our collective is growing across the country, bringing together the DPC physician leaders transforming primary care.

Starting DPC

We connect physicians interested in starting and running their own DPC with employers in areas without DPC.

With expertise in all things DPC, including best-in-class technology, clinical operations, patient experience, and meaningful reporting, we help physicians to launch and optimize their practice. Existing DPC

We connect exisiting DPC physicians and practices with employers and their employees, particularly those with multiple locations or a distributed workforce.

With expertise in partnering with sophisticated employer sponsored health plans, we help existing DPCs to grow their practice through employer patients. By handling the contracting, enrollment, onboarding, and reporting for these employers, 3x5 Health removes any additional burden that seeking out and caring for these lives could bring, while helping you to better convey your value.

The 3x5 Health Collective's comprehensive primary care offering is unified around the monthly fee covering unlimited visits (in-person, phone, text, video), basic primary care procedures, access to labs, access to wholesale medications, and care coordination. In partnership with practices, the collective reports only high value, physician-determined metrics back to employers, including at least utilization and patient satisfaction data. This consistency and simplicity is attractive and essential for employers while still allowing each practice to be inherently local and independent, with the flexibility to customize additional offerings and services in accordance with their own passions or community needs.
